Register at the practice

If you live in our catchment area and would like to register, please complete a registration form for each person registering with the practice. You will be asked for your NHS number (this can be obtained from your previous GP or from the NHS site).

All patients will be asked to complete a new patient questionnaire, to allow us to provide medical care in the interim period, while your records are transferred from your old practice to this one. If you move out of the practice area, you will need to register with a new practice as soon as you move. We can provide information about how to find a new practice.

Any applicants refused registration will be informed of the reason in writing. The very first time a patient registers with a GP practice they will receive a Registration Letter from NHS England which will provide their NHS number.

To register with the surgery online please complete our Patient Registration Form.

If you prefer, you can download a paper version of the patient registration form from there too (listed under ‘Other ways to register’). Once complete please hand into reception.

Alternatively, patient registration forms can be completed in the surgery reception. You will then be booked in to see the Health Care Assistant for a new patient health check.
